To help serve all Fayette County Public School students with meeting KY School Health requirements for the 2017/2018 school year, HealthFirst Bluegrass School-Based Clinics are accepting appointments for the following services:

• School Physical Exams (Early Start, Kindergarten & 6th Grade)
• School Sports Physical Exams
• Required Immunizations (Early Start, Kindergarten & 6th Grade)
• Transfer out-of-state immunization record to a KY immunization certificate with a completed HealthFirst Bluegrass request form. (Five business day processing time, with available pickup on the 6th day.)
