Welcome! If you’re considering making your first appointment with HealthFirst, we’re glad you’re here. We are a Federally-Qualified Health Center. This allows us to provide health care to anyone, regardless of their ability to pay. (Payment is determined on a sliding scale, based on your income.)

How to Schedule an Appointment as a New Patient:
- Call (859) 288-2425. Our call center will get your date of birth, phone number and insurance status. They’ll also help you find the most convenient HealthFirst location.
- Our call center will schedule your meeting with an outreach and enrollment specialist. They’ll help you determine your eligibility for reduced fees. This is usually in-person, at the time of your appointment.
- Fill out this packet.
- Go to your first appointment!
What to Bring to Your First Appointment
- Your Health Insurance card, including Medicaid and/or Medicare cards, if you have them
- Proof of Income (pay stubs, 1099s, W2s or bank statements) —these allow you to qualify for reduced fees
- Government ID, if you have one
- A list of all medications that you (or your child) are currently taking
- Proof of guardianship if you are bringing a child (less than 18 years old) and you are not the minor’s parent
- Medical records (You can bring these or have your previous provider send)
- Immunization records (If you are bringing a child for an appointment)
Sliding Scale Fees
Fees associated with care at HealthFirst are based on family size and household income. Funding is made possible by the Health Resources and Services Administration (HRSA).

Proof of Income Requirements
To comply with federal regulations, we will ask you for proof of household income at your first visit and once a year after that. Proof of income includes:
- Your photo ID
- Your latest income stubs OR…
- Proof of direct deposit for working members of your household
Note: Health Care for the Homeless and School-Based Clinics do not require proof of income for qualifying patients.
